From Kim Kinney at IU Health (formerly Clarian): Hi George. I am trying to collect as much information from Thromboelastograph® (TEG) users as possible. We are in the process of validation to support the Berlin Heart transplant for our pediatric population. I am wondering what other sites use TEG for, who runs the samples, who interprets and what results are entered into the computer system. Any information would be helpful.

We are in the final stages of implementing TEG at our instit
We are in the final stages of implementing TEG at our institution. We have done the reference range verification, and we are now validating our pneumatic tube system. We are performing the TEG in the lab, and only trained MTs will perform the assay. We will initially offer the test for CT and trauma surgery, and we will use the Kaolin clear cup and a heparinase cup. The OR and ICU will have the ability for real-time viewing of the TEG tracing over our network. We will then report the R, alpha, K, MA and estimated percent lysis (EPL) along with our interpretation. The plan is for baseline and post-protamine TEGs, and then another TEG if there is post-operative bleeding.
We also plan to do platelet mapping in the future if this first phase of TEG testing works well. To do platelet mapping, we will need to purchase additional analyzers because platelet mapping requires 4 channels.

We offer TEG testing 24/7 by our trained hematology techs with oversight by the special coagulation lab. We do most of our TEG‘s on liver transplant patients during surgery and they are interpreted by the Anesthesiolologist. The OR suites have computers in the rooms so they can see the results in “real time”. We also perform TEG‘s on ECMO patients, trauma patients and some patients in intensive care. We report the R, angle, MA and %lysis and scan the graphs into the computer system. Reference ranges accompany all parameters. No interpretation is done by the laboratory.

We do TEGs mostly for coronary artery bypass grafts (CABG) on adults. They get a plain and heparinase cup. They are performed in the lab by staff technologist 24/7. We report the same as Dave, except we don’t report CI and don’t provide interpretations, yet.
