ASH 2025 Highlight: Beyond Immunosuppression: Two New Ways to Treat ITP

David Kuter gave this presentation at ASH 2025:

Immune thrombocytopenic purpura is now recognized as a disorder characterized by both reduced production and increased destruction—this understanding drives treatment selection toward achieving complete remission, rather than just disease suppression.

Increase Platelet Production

  • The latest TPO receptor agonist, hetrombopag, now in phase 3 trials, offers a direct pathway to boost platelet synthesis—moving away from suppression toward restoration.

Decrease Platelet Destruction

A diverse arsenal of targeted therapies can redefine our strategy:

  • Anti-CD38 antibodies (daratumumab, mezagitamab)
  • BAFF/BAFF-R inhibitors
  • FcRn inhibitors (efgartigimod)
  • BTK inhibitors (rilzabrutinib, orelabrutinib)
  • Complement inhibitors + immunomodulatory approaches
  • T-cell engagers & CAR-T therapies
