From Susan Lemire, can you confirm that the specimen stability for measuring fibrinogen is 24 hours? Thanks.
Hi, Susan, the attached Scientific Reports article (a Nature publication) gives specimen stability for several coagulation factors and assay results. The study seems conclusive, as the authors used a sufficient sample of 72 specimens and referenced several additional studies. We are always cautious about generalizing results from “asymptomatic patients” to those suspected of hypofibrinogenemia or dysfibrinogenemia, as abnormal fibrinogen may deteriorate at a different rate than normal fibrinogen.

Always one for self promotion: a review I was involved in (Adcock Funk DM, Lippi G, Favaloro EJ. Quality standards for sample processing, transportation, and storage in hemostasis testing. Semin Thromb Hemost. 2012;38:576–85.) reports some studies have found fibrinogen is stable for between 48h–7 days, although the CLSI guideline (H21 A5) states 4 hours. I think that 24 hours would be acceptable based on the literature.
