It should be easy for any lab to validate and offer thrombin times (TT, thrombin clotting time, TCT) to check for presence of a direct thrombin inhibitor, in fact, many labs run the TT as an internal screen for heparin in specimens with unexpected prolonged PTTs. Aniara also offers a thrombin inhibitor assay and Stago is now distributing an Ecarin clotting time kit.

Recently I had a chemistry order on a regular patient of ours. The tube would not clot. I called the physician and was told the patient was on Dabigatran. He lowered the dose and the next week the sample was some better. Any comments. Very few places perform Thrombin Times, time to start?
